Requirements:

1. Come into the office to fill out a Baptism registration form. Please bring:

                * Birth Certificate

                * Marriage certificate of godparents (if married to each other by the Catholic Church)

2. BOTH parent(s) AND godparent(s) must attend baptism preparations.  You will receive updates for classes after registering in the office.
